Qualifying Government Assistance Programs
Below are examples of government assistance programs that may or may not be available in your state. Check the qualifying programs for your state by going to your state page and clicking "Order Now"
- Medicaid
 - Food Stamps (Supplemental Nutrition Assistance - SNAP)
 - Supplemental Security Income (SSI)
 - Federal Public Housing (Section 8)
 - Veteran’s Pension or Survivor’s Pension Benefit
 - Tribally Administered TANF
 - Head Start (Tribal)
 - Food Distribution Program on Indian Reservations
 - Bureau of Indian Affairs General Assistance
 - Income at or below 135% of the Federal Poverty Level
 
Qualifying Customers
					If you are currently receiving benefits from at least one of the above programs, you are probably qualified 
					for Lifeline government home telephone service for low income families. You may 
					also qualify base soley on income if your total household income is at or below 135% of the Federal Poverty Level.
					To get the discounted rate, we will need a signed self certification form from you (except Texas) in 
					addition to placing your order. We will also need proof of your participation 
					in one of the above programs. For those qualifying based on income, you will need to provide 
					proof on household income. If you order your service on-line, the Lifeline 
					certification is included in that process. If you order over the phone, you will need to log into the customer portal 
					using your new account number, and electronically submit a Lifeline certification by following the "update your 
					Lifeline information" links. Or, you can print the paper form, fill it out, and submit it through the mail, fax,

					Texas Customers
					To get Lifeline government assistance discounts in Texas, you must be on file with the Texas Low Income Discount 
					Administration (LIDA), Solix. Once you have submitted your order with us, go to the Texas PUC website and complete 
					the Lite-Up Texas application which is required by the state of Texas. This application will be sent to the Texas 
					LIDA (Solix) who will then notify us of your status. Once you are approved by the Texas LIDA, your phone 
					bill will include the Lifeline discount.
